Through the scene examination and autopsies, the authorities determined that the explosion was caused by a 30kg fire extinguisher.

According to investigators, the U.S. Consulate General hired Quoc An Transport Company to carry 30kg fire extinguishers from the Diamond Plaza (No. 34 Le Duan Street, Ben Nghe Ward, District 1) to the U.S. Consulate General warehouse at No. 51 Nguyen Dinh Chieu Street, Ward 6, District 3.

When the fire extinguishers were brought to the warehouse, Quoc An’s workers were asked to open the exhaust valves of the fire extinguishers. A fire extinguisher exploded and the valve shot to the head of Mr. Duong Trong Han, 33, a native of Dong Thap province, a technical staff of the US Consulate General. Han died on the spot. The incident occurred at 2.50pm.
