The patient, identified only as P., is a Lao national whose child is studying in Vietnam. She chose Hanoi Obstetrics and Gynecology Hospital for treatment after her symptoms worsened.

Around three to four days before being admitted to hospital, P. began experiencing sharp pain in her lower abdomen, accompanied by headaches, fatigue and fever.

As the symptoms became increasingly severe, she decided to make the journey from Laos to Hanoi to seek medical examination and treatment.

A clinical examination found that her uterus had enlarged to a size comparable to that of a woman around four months pregnant.

Ultrasound scans revealed multiple fibroids in both the anterior and posterior walls of the uterus, with the largest measuring 77 by 66 millimeters.

Doctors diagnosed her with multiple uterine fibroids, classified as FIGO L2-5, which were causing abdominal pain and required surgical intervention.

The medical team decided to perform an open total hysterectomy while preserving both ovaries. The procedure was intended to completely remove the fibroids while maintaining the patient's ovarian hormonal function.

During surgery, doctors found numerous fibroids throughout the body of the uterus.

One particularly large fibroid, measuring approximately 7 by 8 centimeters, was located at the uterine isthmus.

Its position made the operation more complex because of its proximity to blood vessels and urinary tract structures.

Through careful dissection, close control of the vascular pedicles and efforts to preserve surrounding structures as much as possible, the surgical team successfully completed the operation without complications.

The patient was in stable condition following surgery and has since been discharged from hospital.
